Eligibility is typically for individuals 55 and older who qualify for nursing home level care but can safely live at home with support.
Services may include primary care, specialist visits, medications, therapy, transportation, and help with daily activities.
Agents can refer qualified clients and support them through the introduction process. Enrollment is handled through the PACE provider.
PACE works alongside Medicare and Medicaid, and for many participants, it becomes their primary source of care and coverage.
